Community Oriented Approach to Coordinated Healthcare

COACH supports the information needs of the network of providers serving Medicaid beneficiaries in the Durham County Carolina Access-II program. COACH is a web-based system that Topsail developed for and in collaboration with the Duke Division of Community Health.

At last count, COACH supports the DCHN Care Management Team, 7 Primary Care Clinics, 3 Urgent Care Facilities, 2 Emergency Departments, 5 Hospitals, and 2 Government Agencies.

The data resides in a SQL Server database maintained by Duke and the web application is hosted on a server also maintained by Duke.
