COACH

Community Oriented Approach to Coordinated Healthcare

COACH is a web-based system Topsail developed in collaboration with the Duke Division of Community Health that supports the information needs of the network of providers serving Medicaid beneficiaries in the Durham County Carolina Access II program.

Currently, COACH connects providers in the MPCC Care Management Teams, 32 Primary Care Clinics, 3 Urgent Care Facilities, 5 Emergency Departments, 5 Hospitals, and 5 Governmental Agencies as they rely on its information to effectively manage their patients' chronic illnesses

The data resides in a SQL Server database maintained by Duke and the web application is hosted on a server also maintained by Duke.
